At Fortify Physical Therapy, LLC (“Fortify”), your privacy and confidentiality are valued greatly. This Privacy Policy applies to information provided to us, whether in person, by telephone, mail, e-mail, our Website (the “Website”), or social media pages, by you, our clients, and prospective clients (hereinafter referred to as “you” and “your”). By visiting the Website, you accept the policies described in this Privacy Policy, and your continued use of the Website after such changes have been posted, constituting your acceptance of such changes.

What Personal Information Do We Collect?

Voluntary Data Collection: We collect your Personal Information that is voluntarily provided to us either directly by you, your employer, or your sponsor, as applicable, in order to provide you with the products and services you have requested. In most cases, it consists of your full legal name, address, email address, and phone numbers. We may also collect your employer or sponsor’s name, driver’s license or other government-issued identification number, date of birth, gender, insurance information (if applicable), and other health related information you deem important to your case and requests. We may use your Personal Information to provide you with health related information, including providing you with responses to requests for physical therapy and/or personal training services, notifications, scheduling confirmations and updates, as well as managing your account, processing bills, and/or personalized mailings (electronic or otherwise) or other communications that you consent to receive, and to prevent potentially prohibited or illegal activity, resolve disputes, and collect fees.

Please note that information provided by you on a voluntary basis through our contact form located at is not sent over a HIPAA compliant server. Any and all health information requested by us through our patient portal login or online health forms found at will be sent over a HIPAA compliant server hosted on a third party website, for which we hold a Business Associate Agreement.

Automatic Data Collection Technologies: We may collect other information, such as your IP address, search terms, and site pages when you visit us online. The technologies we use for this automatic data collection may include cookies and log data.

Cookies are data that websites send to your browser, which may then be stored on your system. We use cookies to better personalize your experience with our Website. We also may use cookies to offer you products, programs, or services. However, cookies alone do not tell us your e-mail address or other Personal Information unless you choose to provide this information to us. We do not currently share your information that we collect for preference-based advertising purposes, and we do not currently have any plans to do so.

Email Communications: We may share your email contact information only with prior expressed consent by you in writing.

Transfer of Assets: We do not sell or lease personal information or provide a list of our customers to third parties. However, if another entity acquires us or all (or substantially all) of our assets, the information we have about you will be transferred to and used by this acquiring entity. Also, if any bankruptcy or reorganization proceeding is brought by or against us, all such information may be considered an asset of ours and as such may be sold or transferred to third parties.

Other Sharing: We may also share your information to investigate, prevent, or take action regarding illegal or suspected illegal activities, to protect and defend our rights, property, and safety of Fortify Physical Therapy, our clients or others, in accordance with our terms and conditions and other agreements.

Third-Party Privacy Policies:  Once your Personal Information has been shared with a non-affiliated third party in accordance with this Privacy Policy, we cannot control how that third party collects, uses or shares your Personal Information and as such, we will thereafter have no responsibility for its use. Protection of your Personal Information in the possession of any such third party shall be governed by the privacy policy of that third party. We strongly encourage you to read the privacy policies linked in third party websites, as Fortify Physical Therapy’s  privacy policy does not pertain to these third parties and these third parties may have policies that differ from Fortify Physical Therapy’s privacy policy.

This Website has not been designed nor intended to be used by anyone under the age of 18 years old. Accordingly, this Website should not be used by anyone under the age of 18 years old. Fortify Physical Therapy specifically requests that persons under the age of 18 years old do not use this Website or submit Personal Information to the Website. Fortify Physical Therapy does not knowingly collect information from minors under 18 years old.  If you are under the age of 18, please do not submit any personally identifiable information to Fortify Physical Therapy.
